PGIMER 2009 Application Form

The prescribed application forms along with prospectus (Brochure of Information) are also available from the office of the undersigned either personally on payment of Rs.500/- for General / OBC / OPH category and Rs.400/- for SC / ST at the counter (Kairon Block, Room No.309) form 10.30 AM to 12.30 P.M. and from 2.30 PM to 3.30 PM on all working days except Saturdays (On Saturdays, forms will be available form 10.30 AM to 12.00 Noon) or by post for which the request must be accompanied With a self addressed thick envelope of size 24 X 15 cm bearing postage Stamp of Rs.30/- and Bank Draft Preferably from any SBI Branch payable at SBI Medical Institute Branch (Code NO.1524) Chandigarh for General / OBC / PH Rs.500/- and for SC / ST Rs.400/- (including category) drawn in favour of the Director, PGI Chandigarh.

However prospectus for D.M / M.Ch wll be available for Rs.500/- only as there is no reservation for the same

a) Application for admission to the course are required to be submitted on the prescribed form enclosed with this prospectus.

b) The application, complete in all respects accompanied with all the relevant documents should reach, on or before the closing date.

The applications received after the closing date will not be considered and no correspondence will be entertained.

c) The fact that application form has been supplied on a certain date-will not be accepted as an excuse for the late submission of the application.

The supply of application form does not ipsofacto make the recipient eligible for admission to the selection tests.

d) Applications on forms other than prescribed for the purpose will not entertained. If any candidate gets an application form for a course different from the one asked for, he/she may submit the same application before the closing date. The application will be treated as an advance copy. The application on the prescribed form will be obtained later.

N.B: There will be negative marking for the wrong. Answers in the theory paper for part II only.
