NCHMCT shall dispatch printed NCHMCT JEE OMR Application Form & Information Brochure 2010 to such candidates for filling it up and sending back to JEE Cell, NCHMCT, NOIDA by 12th April, 2010. Downloaded NCHMCT application without prescribed fee shall not be considered. Demand Draft drawn after 29-3-2010 shall also not be considered for sending the printed NCHMCT JEE 2010 application form and information brochure.
Important Notes for filling up of NCHMCT JEE 2010 Application Form:
Read the instructions carefully before filling the NCHMCT JEE 2010 Application Form.
DO NOT: scribble, smudge, cut, tear or wrinkle the NCHMCT Application Forms or put any stray marks anywhere on the NCHMCT JEE 2010 Application Form .
Paste only recent passport size photograph with date printed. Photograph more than 3 months old shall not be accepted. Do not staple it. (Keep two more copies of the same photograph handy)
Your name, address and signature should be in black ball pen only.
Please note that your name, your parent's/ guardian's name and your date of birth should exactly be same as given in your High school or your first Board/ Pre-University Examination Certificate.
All columns in the Application Form of NCHMCT JEE 2010 should be completed only in English letters.
Incomplete NCHMCT JEE Application Form 2010 or Application Form filled in a language other than English will not be accepted. Don't forget to note down the Application Form number for any future reference or correspondence with National Council. Also keep a photocopy of the filled in ANCHMCT JEE 2010 pplication Form for your future record/ reference.
Mailing your Filled NCHMCT JEE 2010 Application Form:
* How, where and by when to send your completed Application Form Duly completed Application Form, along with Acknowledgement Card and Admit Cards (in triplicate), should be sent to in the pre-addressed envelope.

* Do not staple or paste the Acknowledgement Card and Admit Cards with the NCHMCT JEE 2010 Application Form.
* Ensure that the NCHMCT JEE Application Form 2010 is not folded. Sealed envelope containing filled in Application Form, Acknowledgement Card & Admit Cards be sent through Registered Post/ Speed Post only not later than 29th March, 2010. (NCHMCT JEE 2010 Application sent through private courier services will not be accepted).
* NCHMCT Applications mailed by Registered Post/ Speed Post after 29th March, 2010 shall also not be accepted. (Please indicate code and name of the center of your choice for the written test in the appropriate boxes given in the envelop, also tick the check list indicated on the envelop).
* Acknowledgement Card Write your NCHMCT JEE 2010 Application Form number in the space provided in the Acknowledgement Card. Also write your name and complete postal address in capital letters with PIN Code in the appropriate place. The Council will post the Acknowledgement Card immediately on receipt of your completed Application Form.
Issue of NCHMCT JEE 2010 Admit Card:
* The Council shall issue Admit Card to the candidate indicating roll number, name and address of examination venue enabling the candidate to carry the Admit Card to the allotted venue on the day of examination for appearing in the test for verification by authorized representatives. Candidate should carefully check the Admit Card for all the entries made therein. Incase of any discrepancy, it should immediately be brought to the notice of the JEE CELL in the Council in writing.
Non-receipt of NCHMCT 2010 Admit Card:
* If the NCHMCT JEE 2010 Admit Card is not received by 1st May, 2010 the candidate should access the Council's web site and key in his/ her Application Number to access the required information and can collect duplicate Admit Card from the allotted center of JEE. The candidate will be issued duplicate Admit Card by the Central Test Administrator(CTA) only on the day of examination if the details and photograph match with the original record available at Examination Center.
